Treatment of Cystic Lymphangioma with Topical Use of Bleomycin in Childhood



Abstract
Topical use of Bleomycin as a primary therapy was done in 35 patients with cystic lymphangioma from May 1984 through December 1990 at Department of Pediatric Surgery, Kyungpook National University, School of Medicine.
Total or nearly total shrinkage(Excellent) of the lesions was noted in 24 cases(68.6%), marked or slight shrinkage(Good) in 8 case(22.9%) and no response was noted in the remaining 3(8.5%).
Recurrence of the lesions has not been observed in the follow-up period(1 to years)
Total doses of Bleomycin injection per patient were below 4 mg/kg of body weight.
Repeated injection was given when shrinkage of the lesion was judged insufficient 2 weeks after the first treatment.
As a side effect local inflammatary reactions such as swelling or redness lasting 2 to 3 days were observed in 18(51.4%) patients. Moderate fever(37.5c) was noted in 13(37.1%) patients.
Vomiting and diarrhea were observed in 2 and 1 patient respectively. No respiratory distress was experienced in any case.
Topical use of Bleomycin for cystic lymphangioma can be the first choice in the place of surgical excision and also thought to be safe and simple to perform it as a primary therapy and to be more beneficial than surgical therapy.
